Credit Valley Family Health Team

Credit Valley Family Health Team is one of the Family health teams in Mississauga. This includes a team of doctors, nurse practitioners, nurses, social workers, dietitian, pharmacists and physiotherapist, who provide team-based primary care in the north-west side of Mississauga.

Participants can improve physical activity, increase social connections and foster good mental health in an outdoor setting.
